Claim Process
To ensure your claim proceeds smoothly, please follow these simple steps:

No pre-authorisation needed (You do not need to let Denplan know about a possible claim before you receive treatment) |

Receive your course of dental treatment |

|
|
||
|
Pay your dentist for all treatment received (Denplan cannot settle claims directly with your dentist) |

|
|
||
|
You must obtain proof of treatment from your dentist, showing a breakdown of treatment costs |

|
|
||
|
Complete a new claim form for each course of treatment after your dental appointment
|
||
|
|
||
|
Attach your proof of treatment to the completed claim form and return both to Denplan within 60 days of receiving your course of treatment
|
||
|
|
||
|
You should receive settlement within 5 working days
|
